Pharmacy News

Clinical Pharmacy Consultation:

Prescribers are now able to electronically or manually order a clinical pharmacy consult.  The clinical pharmacist is available Monday-Friday 8 a.m.-4 p.m. and has 24 hours to respond verbally and in writing to the request.  Consults should specify a reason such as polypharmacy, antimicrobial stewardship, adverse drug reactions, drug or laboratory interactions, pain management, pharmacokinetics, high risk of readmission, discharge counseling, etc.  In addition to writing for the consult, clinical pharmacist, Katie Taylor Boyd, PharmD, RPh can also be reached at kboyd@slchospital.org or 568-2835.

Name Change:
Beginning February 1, 2012, DECAVAC (Tetanus and Diphtheria Toxoids Adsorbed (Td) vaccine) will be replaced by the manufacturer to TENIVAC (Tetanus and Diphtheria Toxoids Adsorbed).  TENIVAC will be the only vaccine offered by the manufacturer, however there will still be some DECAVAC available until inventory is depleted.  TENIVAC is indicated for active immunization for prevention of tetanus and diphtheria in persons 7 years of age and older.

REVIEW: Vitamin K Reversal Dosing Guidelines (contact MD for all order changes)

INR Above therapeutic range but less than 5; without significant bleeding:
HOLD WARFARIN. Resume then INR is within therapeutic range.

INR equal or greater than 5 but less than 9; without significant bleeding:
HOLD WARFARIN. Resume at lower dose when INR is within therapeutic range.

INR equal to or greater than 5 but less than 9; rapid correction desired:
HOLD WARFARIN AND GIVE: Vitamin K 2.5mg orally or Vitamin K 2.5mg IVPB if NPO

INR equal to or greater than 9; without significant bleeding:
HOLD WARFARIN AND GIVE: Vitamin K 5mg orally or Vitamin K 5mg IVPB if NPO

Serious bleeding or life-threatening bleeding at any INR (significant drop in hemoglobin of 2 g or greater requiring transfusion)
Serious: hematuria, hematemesis
Life-threatening: intracranial bleed; retroperitoneal bleed
HOLD WARFARIN AND GIVE: Vitamin K 10mg IVPB along with FFP

  • May repeat Vitamin K 10mg IVPB in 12 hours if INR is greater than 3 or if patient still has signs of bleeding per physician assessment

Proton Pump Inhibitors Linked to C difficile Diarrhea
Proton pump inhibitors (PPIs) may be associated with a higher risk for Clostridium difficile–associated diarrhea (CDAD), which can trigger more serious intestinal problems, the US Food and Drug Administration (FDA) announced February 8, 2012. The warning applies to both prescription and over-the-counter (OTC) PPIs, including the drugs LSCH has on formulary - Protonix (pantoprazole) and Prevacid (lansoprazole). The FDA is advising clinicians to consider a diagnosis of CDAD for patients taking PPIs who have diarrhea that does not go away. As a precaution, patients should take the lowest dose of a PPI for the shortest duration that is appropriate for the condition being treated. The agency said it also is studying the risk for CDAD in patients taking histamine H2 receptor blockers, prescribed for conditions such as gastroesophageal reflux disease, ulcers of the stomach and small intestine, and heartburn.   Acute Inpatient Hospice Beds

Clinical News - Dormar Hospice in service Beginning in March 2012, SLCH in conjunction with Hospice of Orange and Sullivan, will now have 2 acute inpatient hospice beds.  The beds will be located on 4 North. The patient must be enrolled in hospice and meet one of the following criteria:

  • Imminently dying
  • No beds available at the inpatient hospice house
  • Require complex symptom management
  • Does not have the financial means for the inpatient hospice house or
  • Ventilator dependent or high flow oxygen required

Additional training and education will be provided to the nursing team.  Hospice of Orange and Sullivan will also provide additional support services while the patient is an inpatient
If you are interested a brief educational session will be provided by Dr. Domer on Hospice and end of life care on February 23, 2012 at 8 a.m. in Conference Room C

Dr. Domer will also be speaking at the June quarterly medical staff meeting

The HIM Incomplete medical record room is now paperless!  Thanks to Filomena Marchione, RHIT, Deficiency Coordinator, for assisting the medical staff with the transition from paper to electronic deficiencies and the entire medical staff for being so cooperative with the change. 

A special thank you to Dr. Christos C. Lambrakis, MD, for completing the last paper medical record at SLCH.

JetBlue Airways donates quilts to pediatric patients


JetBlue Airways and Soaring Words, an organization dedicated to inspiring perseverance in chronically ill children and their families, has donated several handmade quilts for pediatric patients at SLCH. Each quilt is individualized with either a hand painted or embroidered cheerful message.

Thank you to JetBlue for donating these wonderful blankets again this year!

Provident Bank Donates Grant to Palliative Care Program


The Palliative Care program at SLCH has received $25,000 from Provident Bank. This generous donation will contribute to the Palliative Care program’s efforts to preserve the quality of life for patients with terminal illnesses.

“This program enables us to focus on our continuum of care model that facilitates holistic treatment for the patient and family during a challenging time,” said Sue Sullivan, Vice President, Advancement and Government Affairs. “We appreciate Provident Bank’s support in helping us serve our community.”


Scholarship Opportunities Announced



St. Luke’s Cornwall Health System Foundation will soon award annual scholarships to graduating high school seniors entering an accredited university or community college in the fall of 2012. The scholarships will be provided in recognition of scholastic excellence and volunteerism.

Kathy Van Orden is the February Star of the Month!

The February Star of the Month, Kathy Van Orden, Materials Coordinator, is an exemplary employee in many ways.

“Kathy has an excellent rapport with all of our outside vendors,” said Susan McKean, SLCH Materials Management. “She is always the first employee to assist me with any projects that we need to handle, and is just a genuinely good person.
